First European Cases 

Hashim U. Ahmed, Professor in Urology & Consultant Urological Surgeon, performed the first ever European TP Pivot Pro™ cases at Charing Cross Hospital, London. Here, Hashim successfully used the TP Pivot Pro to perform prostate biopsies procedures using the freehand, transperineal approach.

“The histology on all three cases came back exactly as I would want it to. So, all the targets came back as showing cancer; it means we didn't miss”, says Hashim.

According to Hashim, using TP Pivot Pro™ in the first couple of cases has been great, and he found the device intuitive to use with a learning curve that is not overly steep. While attaching and reattaching the device to an ultrasound probe - as with any needle guide - requires some practice, Hashim emphasizes that TP Pivot Pro™ is straightforward to use.

“We've been waiting for something like this to come onto the market that differentiates itself from using a grid or the other devices. And the beautiful thing about this is a number of features, really. First of all, the various steps and levels that you can use. Secondly, the ability to angle, and that allows us to reach anywhere in the prostate that we want to target. A clear case for transperineal biopsy  
Hashim highlights the clear benefits of transperineal over transrectal biopsy, as strong evidence shows significantly lower sepsis and infection rates. With proper training and adherence to standardized protocols, transperineal biopsies can be performed under local anesthesia, ensuring patient safety and comfort. While antibiotic prophylaxis remains necessary in major cities like London, smaller towns may manage without it, says Hashim. Avoiding infections not only prevents life-threatening complications but also reduces reliance on strong antibiotics, supporting better antibiotic stewardship. Furthermore, Hashim adds, transperineal biopsy provides superior access to apical and anterior lesions, making it a more effective diagnostic tool.
